Need a copy of your SonderMind records? Here's what you need to know.


SonderMind maintains strict policies regarding the release of information to ensure the security and privacy of mental health records.


For the fastest processing, all requests for records should be submitted by the client or their personal representative through the SonderMind Client Portal.


Properly submitted record requests are typically processed within 5-7 business days but may take up to 30 days to process.

Clients and Client Personal Representatives: 

  • Login to the SonderMind Client Portal 
  • Navigate to the ‘Account’ tab
  • Scroll down to and select ‘Records Request’
  • Complete the form

Note: If your client account has been deactivated, please email [email protected]

Third parties: Please have the client submit the form via the Client Portal, using the steps provided above and including your contact information in the form to authorize the release of their records directly to you.


If the client is unavailable (e.g. deceased or incarcerated), please note that SonderMind client records are subject to heightened protections beyond standard medical records. Third party requests received directly to [email protected] and not through the client portal will be reviewed subject to SonderMind’s obligations under applicable law and may take longer to process.


Insurance Companies and Payors should not use the Client Portal or submit requests to [email protected]. Records requests for payment, treatment, or operations purposes under a valid contract with SonderMind or a SonderMind payor, should be submitted via email to [email protected] and include a specific due date.
